To identify and understand the different types of communication failings that can lead to substandard care and in some cases, patient harm. Specifically, this review is intended to address the following research questions:
1. What are the most commonly reported/studied types of communication breakdown that lead to substandard care or harm?
2. What parts of the care process do these correspond to and which professional groups do they affect/involve?
3. Is there consensus, evidence or a shared view on the main contributory factors that lead to their occurrence?
